This site is a design demonstration for a practice that does not exist. It does not set analytics or advertising cookies, and it does not collect, transmit or store personal information. The consultation request form on this site shows a confirmation on screen and sends nothing; no message is delivered and no record is kept.

A live practice website built on this design would state here exactly which details its forms collect (typically name, contact details and the reason for the visit), where they are sent, and how long they are retained.

A dental practice handles personal health information and is bound by the privacy legislation of its jurisdiction, such as HIPAA in the United States and PIPEDA or provincial health-privacy law in Canada. Clinical records are never collected through a website form. Anything a patient shares online is used only to arrange a visit and is handled with the same care as a record made in the chair.

This demonstration loads no third-party scripts, embeds or fonts from external servers. A live site would list any service that receives data on the practice’s behalf, such as an online booking provider, an email service or website analytics, and what each one receives.

You may ask a practice what information it holds about you, request a correction, or ask that your details be removed from its marketing contact list. Requests are normally answered within thirty days.
